  1. What medical forms does a student at SRMHS need to participate in athletics?
    • Physical
      The North Carolina High School Athletic Association requires a physical examination to be eligible for interscholastic athletics. This physical is valid for 1 year (365 days) from the completion date.

      A licensed medical physical must complete the Wake County Physical Form. Physicals may be performed by your primary physician, but the enclosed form must be used!

    • Student–Athlete Information Form
      This form provides important information that would be necessary in the event an emergency should arise. Health History Form – This form must be completed as thoroughly as possible to get a detailed student–athlete health history as well as a family health history.

    • Athletic Insurance Form – The students and parents must sign this form, thereby indicating they understand the Wake County Athletic Insurance Policy.

    • Permission to Treat, Consent and Authorization for Release of Medical Records/Information, and Acceptance of Risk – This form has three (3) parts. Please read carefully!
      1. The permission–to–treat form allows the medical staff at SRMHS to treat your child for any injury or emergency that occurs as a result of athletic participation.
      2. The authorization for release of medical records allows the medical staff at SRMHS to keep confidential medical records on your child and to release medical information on your child to health care providers only.
      3. The acceptance of risk form assumes that the athlete is aware of the risk involved with participating in athletics and he/she agrees to follow the advice of the SRMHS medical staff.

  3. Why is an ATC necessary in a high school?
    ATC intervention reduces re–injury rates reduces time loss from non–physical injuries. Recent data showed that patient satisfaction rates are above 96% when treatment is provided by a Certified Athletic Trainer. "The daily utilization of services provided by a Certified Athletic Trainer to high school athletes can result in the reduction of accident insurance premiums for the high school," says John Jay, President of Scholastic Insurance, INC.
